Overview

Published with the inputs from Dr Sylesh Aikot, Surgical Gastroenterologist,Baby Memorial Hospitals- Kozhikode

A deceased donor liver transplant is a surgery where a diseased or failing liver is replaced with a healthy liver from a donor who has passed away and consented to organ donation. This life-saving procedure is often the only treatment for patients with end-stage liver disease (ESLD), liver failure, or certain types of liver cancer.

What is a deceased donor liver transplant?

It is a liver transplant in which the organ is taken from a donor who has died, with the family’s consent, and transplanted into a recipient whose liver no longer functions properly. The donated liver is healthy and able to restore normal liver function.

Who needs a liver transplant?

A transplant is usually recommended for patients with:

  • End-stage liver disease from cirrhosis (alcohol-related, viral hepatitis, NASH)
  • Acute liver failure
  • Hepatocellular carcinoma (HCC) within transplant criteria
  • Certain metabolic or genetic liver diseases
Is liver transplant safe?

Yes. It is a complex but well-established procedure with very high success rates in specialized transplant centers. Risks include bleeding, infection, rejection of the organ, or complications from immunosuppressive medicines, but these are closely monitored.

What is the success rate of deceased donor liver transplant?

Success rates are excellent. On average:

  • 1-year survival: over 85–90%
  • 5-year survival: about 70–80%

Outcomes depend on the patient’s overall health, liver disease severity, and post-transplant care.

How long can you live after a liver transplant?

Many patients live decades with a healthy, functioning transplanted liver, provided they take lifelong medications (immunosuppressants) and follow medical advice.

How is a deceased donor liver transplant performed?
  • The diseased liver is surgically removed.
  • The healthy donor liver is transplanted in its place.
  • The surgery takes about 6–12 hours and is followed by an ICU stay for close monitoring.
What is recovery like after a liver transplant?
  • Hospital stay usually lasts 2–3 weeks.
  • Immunosuppressive medications must be taken lifelong to prevent rejection.
  • Patients gradually return to normal activities in 3–6 months.
  • Regular follow-up is essential to monitor liver function.
What lifestyle changes are needed after a transplant?
  • Taking medications exactly as prescribed
  • Eating a balanced, liver-healthy diet
  • Avoiding alcohol completely
  • Preventing infections with good hygiene and vaccinations
  • Staying physically active as tolerated
